Richland County restaurant inspections: critical violations July 22-26

The following restaurant inspections with critical violations were conducted by Richland Public Health between July 22 and July 26:

• Mickey Mart #22, 189 Plymouth St., Plymouth, July 22. Food employee(s) not washing hands in approved handwashing sink or automatic handwashing facility (critical, corrected during inspection). Employee observed washing hands in three-compartment sink. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Raw eggs observed stored above ready-to-eat foods in walk-in cooler. 

• Arby's #7585, 689 Lexington Springmill Road, Mansfield, July 22. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed foods in Traulsen freezer to be unprotected. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed dried food on a lean lid coming out of the dishwasher. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed a sanitizer bucket at below 100 ppm. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Observed sliced roast beef in hot holding to be between 125-131 degrees. Person in charge (PIC) voluntarily destroyed 18.5 ounces of roast beef. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed ranch dipping sauce at 50 degrees and a can of whipped cream at 57 degrees being held without refrigeration. PIC voluntarily discarded. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed peppercorn ranch at 49 degrees and gyro at 45 degrees on make line. When finished prepping, place product back into cooler until product reaches 41 degrees or less. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed a bottle of lubrication on top of equipment. Presence of live insects, rodents and other pests (critical). Observed live gnats on the floor next to the water heater. PIC stated pest company has been contacted and a bug light has been ordered. Correct by re-inspection Aug.1. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed the dining room fountain drink machine not to be air gapped. Correct by Aug. 1. 

• Wal-Mart #5471, 2485 O'Possum Run Road, Mansfield, July 22. Handwashing sink not accessible (critical). Observed a box and boxing paper located on hand sink in Deli Department. 

• Panchos Tacos, 1344 Lexington Ave., Mansfield, July 25. PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical, corrected during inspection). Sanitizer strength was at 0 ppm in several buckets. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected during inspection). Rice in hot holding was 118 degrees. Chicken and sliced tomatoes in make table were not held with an internal temperature of 41 degrees. PIC voluntarily discarded. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Employees present at time of inspection had not been informed of their responsibility to report in a verifiable manner. Handwashing sink is being used for purposes other than handwashing (critical repeat, corrected during inspection). Employee observed using hand sink in front to rinse out rag. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Container of shrimp observed in two-door cooler was not covered. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical repeat, corrected during inspection). Chlorine sanitizer in buckets was at 0 ppm. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Can opener blade had visible residue. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Rice in steam well had internal temperature of 118 degrees for an undetermined amount of time. PIC voluntarily discarded 5 pounds of rice. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Container of sliced tomatoes in make table had internal temperature of 47 degrees for an undetermined amount of time. PIC voluntarily discarded 3 pounds of sliced tomatoes. Raw chicken in make table had internal temperature of 50 degrees. PIC voluntarily discarded 5 pounds of chicken. Improper use of a poisonous or toxic substance (critical repeat, corrected during inspection). Pest control strips observed in use by clean utensils. Presence of live insects (critical repeat). Flying insects observed in food preparation areas. Please provide most recent pest control invoice from licensed pest control applicator and have insects removed before re-inspection. 

• In-N-Out Mart #103, 300 N. Mulberry St., Mansfield, July 25. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed no Quat sanitizer in three-compartment sink. 

• Applebee's Neighborhood Grill & Bar, 145 Stander Ave., Mansfield, July 25. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed food debris inside a salad bowl in the pull-out cooler drawer. 

• Circle K #4705374, 2094 Park Avenue West, Mansfield, July 25. Person in charge did not ensure their employees are properly trained in food safety (critical, corrected during inspection). Pizza pans are not being washed. PIC unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). Education provided for critical violations. Manual wash solution not being maintained at 110 degrees or above (critical, corrected during inspection). Observed manual wash water in three-compartment sink to be 99 degrees.

• Pulp Juice and Smoothie Bar, 994 Lexington Springmill Road, Ontario, July 26. PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected during inspection). PIC unable to verbalize proper cold-holding temperature for TCS foods. PIC unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed large amount of washed oranges stored on top of juicing machine unprotected, exposed to contamination. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed all dry mixes in containers on prep counter, jar of peanut butter, and container of chia seeds to be uncovered/unprotected. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed lid on freezer at front prep area to be open while not in use, as well as food inside freezer uncovered. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed open bag of greens in cooler unprotected. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed multiple foods (cut carrots, cucumbers, ginger, celery) in containers in cooler below juicing machines to be uncovered/unprotected.
