The following restaurant inspections with critical violations were conducted by Richland Public Health Oct. 7 - Oct. 10:

● Texas Roadhouse, 515 N. Lexington Springmill Road, Ontario, Oct. 7. Person in charge (PIC) not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical, corrected during inspection). Employee not sanitizing food thermometer between use. Handwashing sink not accessible (critical repeat, corrected). Observed pitcher and knife stored on handwashing sink in bakery area. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed multiple foods in storage throughout facility to not have lids/covers to protect from possible contamination. Chemical sanitizer other than chlorine, iodine, or a quaternary ammonium applied improperly (critical, corrected). Observed working bucket of sink and surface sanitizer in chicken prep area and bar area to be under strength. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed clean cutting board hanging in clean storage in dishwashing area to contain visible residue. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical). Observed chlorine sanitizer at dishwasher to be 0 ppm. Facility to manually dip clean utensils/equipment until dishwasher is repaired. Correct by Oct. 17. TCS foods not being cold held at the proper temperature (critical, corrected). Observed garlic stored in oil to be at 68 degrees. PIC discarded during inspection. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed house-made Italian dressing and lemon butter not consumed or discarded by date marked on container. PIC discard foods listed. Improper storage of poisonous or toxic materials (critical, corrected). Observed spray bottle of cleaner stored on shelf above clean equipment/utensils. Presence of live insects (critical). Observed gnats near syrups in bar area. Thoroughly clean area and monitor for further activity. Corrected by Oct. 17.

● Shiloh Elementary, 420 Trux St., Plymouth, Oct. 7. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Copy provided to PIC. Improper washing of hands and arms (critical, corrected). Food workers observed touching contaminated surface of sink handle with washed hands. Insufficient air gap between the flood rim and the water supply inlet (critical). Water supply inlet was below flood rim. Correct by Oct. 14.

● Subway Sandwich Shoppe #38123, 359 N. Lexington Springmill Road, Ontario, Oct. 7. Presence of live insects (critial repeat). Observed small flying insects near fountain beverage station, back storage room and near mop sink. At reinspection, observed no flying insects near fountain machine, however flying insects continued to remain in back storage area. Remove all presence of small flying insects or contact licensed pest control company for treatment by next standard inspection.

● Plymouth-Shiloh High School/Middle School, 400 Trux St., Plymouth, Oct. 7. Insufficient air gap between the flood rim and the water supply inlet (critical). Double-compartment sink had water supply inlet below flood rim. Minimum air gap must be at least one inch difference between flood rim and water supply inlet, above flood rim. Correct by Oct. 14.

● Speedway #3535, 838 Ohio 97, Bellville, Oct. 7. PIC unable to demonstrate knowledge by having no critical violations (critical). Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). PIC unable to locate signed copies of employee illness reporting agreements. Provided a copy of RPH's employee illness reporting agreement form. Food employee(s) did not wash hands when required (critical, corrected). Observed a food employee touching disposable gloves before washing hands. Handwashing sink not accessible (critical, corrected). Observed cardboard boxes sitting on the floor in front of the handwashing sink. Foods from unapproved sources (critical). Observed farm-raised brown eggs in a yogurt box on the bottom shelf inside the walk-in cooler. PIC stated eggs are a personal item. Foods from unapproved sources shall not be stored in a licensed food facility. Correct by Oct. 14. TCS foods not being hot held at the proper temperature (critical, corrected). Bagged cheese and chili sauce in the warmer had an internal temperature of 97 degrees. PIC voluntarily discarded cheese and chili sauce. TCS foods not being cold held at the proper temperature (critical, corrected). Sliced American cheese in the reach-in cooler at 45 degrees was observed sitting on the counter for an extended period of time. PIC voluntarily discarded cheese. Ready-to-eat, TCS food not properly discarded when required (critical). Boxes of French vanilla and mocha flavors in the iced coffee dispenser had an open date of Sept. 19. Refrigerated TCS food items shall be held for no more than seven days once opened. Correct by Oct. 14. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed containers of hot dogs in the walk-in cooler with a prep date of Oct. 6 and a use by date of Oct. 21. PIC corrected use by date to Oct. 13 to hold for no more than seven days. Missing and/or incomplete allergen declaration(s) (critical). Ingredient list for donuts in the self-service display case not posted for customer viewing. Correct by Oct. 14. Missing and/or incomplete allergen declaration(s) (critical). Lindt Lindor chocolate truffles in the retail display bin did not have a complete list of ingredients. Correct by Oct. 14. Food display not properly protected from contamination by consumers (critical repeat, corrected). Observed unwrapped apples in a retail display shelf. PIC removed apples from sale.

● El Charrito, 80 W. Hanley Road, Mansfield, Oct. 10. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed chicken over beef in the walk-in cooler. Walk-in cooler should be organized in descending order of cook temperature. Equipment food-contact surfaces or utensils are unclean (critical). Observed the cutting board of the make line to be stained and with deep cuts/grooves. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed dirty dishes on the clean dish area. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed bread maker not in operation with visible residue. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed utensils and thermometers against the prep area wall and the electrical wiring. Improper reheating of food for hot holding (critical, corrected). Observed tamales to be reheating for two hours and have yet to reach internal temperature of at least 165 degrees. PIC had 10 tamales discarded. TCS foods not being cold held at the proper temperature (critical, corrected). Observed cold-holding temperature of half-and-half creamer to be 58 degrees. Half pint was discarded by PIC. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed open package of hot dogs to be in reach-in cooler under make line to be opened without proper datemarking. PIC voluntarily discarded the package.

● Burger King Restaurant #585, 851 Ohio 97 W., Bellville, Oct. 10. Improper use of time as a public health control - 4 hours (critical, corrected during inspection). Observed cheese on make line with timestamp of 7 a.m. and was past four-hour threshold. Employee discarded cheese. Observed time stamp label on cut lettuce to not have time marking. Employee verbalized time lettuce was placed on make line and appropriately labeled during inspection.

● McDonald's - Trimble Road #7342, 540 N. Trimble Road, Mansfield, Oct. 10. Improper use of time as a public health control - 4 hours (critical, corrected during inspection). Observed a container of butter packets past the time stamp for 12:57 p.m. PIC voluntarily discarded butter packets at the time of inspection.
